Most scope damage happens in the gaps between cases, which is also where the fix lives. One academic medical center added routine visual inspections and watched annual repair costs drop from more than $1.2 million to about $724,000.

Hot take: arguing about whose fault it is doesn't get a single instrument any cleaner.

Robin Evans, BSN, RN, RNFA, CRCST, CHL, has seen the same argument play out in hospital after hospital. Sterile Processing says the OR isn't doing point-of-use care. The OR says cleaning instruments is Sterile Processing's job. Neither side is entirely wrong, and neither side is entirely right.

Her take: the real question isn't who's at fault. It's why the process keeps failing and what a hospital plans to do about it.

Across the country, parents are getting kids back into the swing of a new school year. Back to school means back to routine, and perioperative departments should be feeling the same reset.

The busiest surgical months are approaching, largely because patients schedule procedures once they meet their annual deductible. July is typically the slowest. That gap is why late summer matters: it is the window to catch problems before they turn into fall bottlenecks.

A parent would not send a child back to school without the right supplies. An OR should not head into its busiest season without the right equipment, instruments, and staffing in place either.
