Guanfacine is getting another look in adult ADHDâand new 2026 research is adding to what we know about its use in adults.
Unlike stimulant medications, guanfacine works on alpha-2A adrenergic receptors, helping regulate signaling in the prefrontal cortexâthe area involved in attention, impulse control, emotional regulation and executive functioning.

A new 2026 real-world adult study followed patients taking extended-release guanfacine for up to a year, adding important information about its effectiveness and safety outside of a traditional clinical trial.

It can cause sleepiness, dizziness, lower blood pressure and a slower heart rate, which is why dosing, blood pressure and response should be medically monitored.
