That twitch isn’t just a cool party trick. There’s real neuroscience behind it. 🧠

When a dry needle contacts an active myofascial trigger point, it can produce a Local Twitch Response (LTR) — an involuntary, rapid contraction of the taut muscle band.

⚡ Active TrPs contribute to sensitization of spinal dorsal horn neurons and brainstem pathways — meaning a knot in your patient’s upper trap may be driving pain that presents far from the source

🧲 Dry needling works across multiple levels: reducing peripheral nociception at the TrP, decreasing dorsal horn neuron excitability, and modulating pain-related brainstem areas including the periaqueductal gray — the brain’s own pain-suppression center (Niddam et al.; Fernández-de-las-Peñas et al., 2019)

🔄 Research also supports local vasodilation following needling, which may help flush the ischemic, pro-inflammatory biochemical milieu that keeps a trigger point active

The LTR itself? The literature is still evolving. Some research suggests eliciting it may not always be necessary for clinical outcomes — what matters more is WHERE you needle, HOW you assess the tissue, and whether your treatment is integrated into a broader clinical reasoning framework.

Most clinicians miss this about the piriformis. 👇

The piriformis is almost always on your differential for posterior hip and sciatic-type pain — but its insertion tells a different story than most textbooks show.

In external rotation, it behaves as a hip external rotator. In hip flexion past 60°, it flips — and actually becomes an internal rotator.

Myth 1: You must feel a twitch response for dry needling to work.
False. Twitch responses can indicate you’ve reached a trigger point, but research shows therapeutic benefit occurs without them. Chasing the twitch can increase patient discomfort unnecessarily.

Myth 2: Dry needling is just acupuncture with a rebrand.
The philosophy, point selection, and physiological targets are entirely different. DN is rooted in Western anatomy and neuromuscular science — not meridian theory.

Myth 3: It’s only for trigger points.
DN has well-documented applications for tendinopathies, periosteal conditions, neural desensitization, and motor retraining. If you’re only using it for trigger points, you’re leaving clinical results on the table.

Myth 4: More needles = better outcomes.
