Over the muscle or under it. Both are real options. ✳️

An implant can sit in front of your chest muscle or behind it, and that choice changes how you look, how you heal, and how your mammograms read for the rest of your life.

Behind the muscle, the implant has a layer of your own tissue over the top of it.

The upper edge is softer and less visible, and there is more coverage if you are thin.

The trade is a tighter early recovery and, in some women, movement in the breast when the muscle flexes. ⚠️

In front of the muscle, the implant follows the shape of the breast more directly and recovery tends to be easier.

The trade is less coverage, so if there is not much tissue to begin with, the edges of the implant can become visible over time. ⭐

There is also a middle approach where the muscle covers the lower portion only. ➡️

None of these is the correct answer for everyone.

It comes down to how much tissue you have, how much sag, and what you do with your body.

If a surgeon only ever does 1 of them, ask why.

The order a mommy makeover follows ⚕️

A mommy makeover is usually more than 1 procedure done in a single operation. The sequence is not random.

1. Breast work often comes first, while the anesthesia team and I are freshest and precision matters most

2. The tummy tuck and muscle repair typically follow. That portion takes the longest and asks the most of the body ⏱️

3. Liposuction to the surrounding areas is usually last, refining the overall shape once the major work is done

4. Positioning changes happen between steps. You are physically repositioned on the table, and that is part of why total operating room time adds up 🔄

The plan is built around safety and precision, not around getting you off the table faster.

It is also why 2 surgeons quoting the same procedures can quote very different operating times.

⚠️ Longer time under anesthesia carries real risk, which is exactly why some patients are staged into 2 operations instead of 1. Sequence and total operative time are decided case by case. This is general education, not medical advice.

The breast lift scar took 100 years to shorten ⚕️

The first breast lifts on record were performed in France in the late 1890s. Surgeons could restore the shape almost immediately 🔬

2 problems took the next century. Keeping the ni**le alive while moving it, and closing the skin without leaving a mark no patient would accept 🧵

The 1960s solved the first. Strömbeck in 1960, Skoog in 1963 and McKissock in 1972 developed the pedicle techniques that keep the ni**le attached to its own blood supply while it is repositioned. That principle has not changed since 🩺

The second problem took longer and is still a trade rather than a solution. Wise standardized the anchor incision in 1956, reliable and long. Lassus began shortening it through the 1960s, and Lejour popularized the lollipop pattern in the 1990s ✂️

Every one of those patterns buys shorter scar at the cost of how much lift it can deliver, which is why none of them replaced the others 📐

What changed most recently is the selection. Since Regnault published his sag grading in 1976, the pattern gets matched to the patient's actual grade rather than to whatever the surgeon does by default 🕰️

⚠️ Every breast lift leaves permanent scars, and scar appearance varies significantly between patients. No technique eliminates scarring. Only an in person exam with a board certified plastic surgeon can determine which pattern is appropriate for you.

Which of the 3 are you? ⚡

Loose skin on the abdomen is 1 complaint with3 different answers, and picking the wrong one is how patients end up disappointed by a technically well done surgery.

If your skin is tight and it is only fat, liposuction alone may do it. ⭐

If the loose skin sits below the belly button only, a shorter incision and a smaller lift can work. ✅

If the looseness runs above and below the belly button, and especially if the muscle wall has separated, that is the full operation with the longer scar and the muscle repair. ⚠️

And if you have lost a large amount of weight and have excess skin running side to side and up and down, sometimes the incision has to go vertically as well as horizontally. ❗

Nobody wants to hear that last one.

A vertical scar is a real trade, and I say so plainly.

But there are bodies where the horizontal incision alone cannot remove enough, and pretending otherwise leaves the patient with a result she paid for and does not want.

Ask your surgeon which of the 3 you are, and ask them to explain why not the smaller one.
