PMOS affects around 1 in 10 women of reproductive age.

How is PMOS diagnosed?

PCOS is diagnosed when 2 out of 3 features are present, after excluding other causes:

🔹 Ovulatory dysfunction
Irregular or missed periods.

🔹 Increased androgens
Hirtuism/excess body or facial hair, acne, or raised androgen or testosterone levels on blood tests

🔹 Ultrasound findings
Polycystic appearing ovaries on ultrasound

👉 You do NOT need to have polycystic ovaries on ultrasound to have PMOS.

PMOS can affect periods, skin, metabolism, and fertility—but symptoms vary from person to person.

Recognising the signs can help with earlier diagnosis and appropriate management.

Taking a GLP-1 weight loss injection like Mounjaro or Wegovy? This is something most people dont know..

These medications slow stomach emptying, which can reduce the absorption of oral medications. That means some oral hormones may not work as effectively, including:
Oral HRT, Oral progesterone/progestogens, the combined contraceptive pill, the progestogen-only pill.

This doesn’t mean everyone needs to stop these medications, but it does mean you may need to adjust your dose of your hormonal pill. In many cases, fully transdermal HRT (patches, gels or sprays) aren’t affected in the same way because they bypasses the gut.

If this applies to you, speak to your GP or menopause specialist.

Can you take HRT after a blood clot or stroke?

I saw a 48 year old lady who had had a stroke last year, she was struggling with brain fog and assumed this was all from her stroke. She also had joint pains, fatigue and night sweats and it was clear she would benefit from HRT. She hadn't realised this was even an option!

For years, women who had a DVT, PE, or stroke were told HRT was completely off the table. This is no longer the case!

According to British Menopause Society (BMS) guidelines, a history of clots doesn’t mean you have to suffer,  your care just needs to be personalised.

Here is what the evidence shows:
• Transdermal estrogen (patches, gels, or sprays) absorbs through the skin and has a neutral effect on clotting. It does not raise your risk.

• The Right Progesterone: If you need progesterone, body-identical options like micronised progesterone are much safer than traditional synthetics.

You do not have to live with severe hot flushes, insomnia, or brain fog. You just need expert, individualised care.

I work with women to provide personalised care to review your medical history to find a safe, tailored treatment plan that protects your health while relieving your symptoms.

Is your HRT making you feel worse for part of the month?

You dont have to struggle or give up on HRT. modifications can help.

● Switch to Micronized Progesterone: Body-identical versions are typically much better tolerated than older synthetic progestogens.

● A different dose or different form of Progestogen can help.

● Vaginal Route - This can bypass the liver, reducing the metabolites so is better tolerated ( discuss this with your doctor first)

● The Mirena IUD This delivers progestogen locally to the uterine lining. It keeps blood levels much lower.

If your HRT is causing a mental health dip, your regimen needs adjusting.
