We are proud to welcome Chelsea Gerena as our new Nurse Navigator here at Cedar Crest Post Acute!
Chelsea brings a wealth of experience and a deeply rooted passion for patient care. Her healthcare journey began as a hospice volunteer while working in retail management—an experience that inspired her to pursue her CNA certification. From there, she built a strong foundation working in long-term care and rehabilitation before earning her LPN in 2016.
Since becoming a nurse, Chelsea has gained extensive experience across the post-acute care continuum, including long-term care, short-term rehab, personal care, and home health. She has held a variety of impactful roles such as Charge Nurse, Assistant Director of Nursing, Assistant Unit Manager, Clinical Support Specialist, and Home Health Account Executive—where she focused on education, care coordination, and helping patients transition safely home.
Chelsea’s passion for nursing is personal. She was inspired by helping care for her grandparents during their cancer journeys and has seen firsthand how meaningful compassionate, quality care can be for patients and families.
She is bilingual in English and conversational Spanish and values the ability to connect with patients in their preferred language, helping them feel comfortable and supported throughout their care.
Chelsea prides herself on her compassion, critical thinking, and ability to balance both patient-centered care and system-level improvements. She is especially passionate about improving communication, education, and coordination throughout the healthcare journey.
