Luke’s Transitional Care Center is helping patients recover from illness or surgery and go home faster. Sometimes after an injury, surgery, or illness you no longer need to remain in the hospital but you are not confident or strong enough to return to your home. We are your bridge between a hospital stay and going home so that when you do go home you are successful and have a reduced chance of reh

ospitalization. Our caregivers and staff are committed to delivering the care you need when you need it. Our highly-skilled team of providers, nurses, social workers, and therapists design a personalized care plan for you with input and communication from the hospital doctors and your primary doctor. From daily visits by a nurse practitioner to a custom rehabilitation program geared at strengthening the body, all services are uniquely tailored to help you meet your individual health goals. Following a qualifying hospital stay, our services may be covered by Medicare. Private pay is also accepted for individuals wanting skilled nursing services for a short time prior to returning home.

Today was the final day of CNA Week, and we wrapped up an amazing week full of fun, laughter, and celebration! 🤠❤️

Today’s theme was Western Day, and staff had a great time dressing up in their cowboy hats, boots, and western wear.

Thank you to all of our incredible CNAs for everything you do each and every day. We appreciate all your hard work, dedication, and compassionate care! We had so much fun celebrating you all week long! 🐴⭐

We are continuing to enjoy CNA Week here at TCC! 🎉

Today’s theme was “Through the Decades” dress up day, and our staff had so much fun showing up representing different eras through the years! From groovy outfits to classic throwback styles, everyone looked amazing and helped make the day extra fun.

We have also been having a blast throughout the week with different relay games and activities for staff to enjoy together! 😊

We're having a blast celebrating CNA Week with themed dress-up days! 🚢🌺
Today's theme was Cruise Ship Day, and our staff had so much fun dressing up and pretending they were setting sail on a tropical cruise. To make the day even more special, Chef Jacob prepared delicious Hawaiian-inspired appetizers and refreshing tropical drinks for everyone to enjoy.
A huge thank you to our incredible CNAs and staff for all they do each day. We love finding fun ways to celebrate and show our appreciation during CNA Week! 🩵⚓🌴

We spent the afternoon celebrating two of our amazing staff members with a special baby shower! 👶🩷🩵

By the end of June, our TCC family will be welcoming two new little bundles of joy, and we couldn't be more excited for these parents-to-be. It was wonderful to come together, share laughs, enjoy treats, and shower them with love and well wishes.

Our residents had a wonderful time this week getting creative with seashell crafts! 🐚🎨 Everyone enjoyed designing their own unique creations while spending time with friends and expressing their creativity. It was a fun-filled activity that brought plenty of smiles and beautiful artwork. Great job to all of our talented residents! ❤️
