St. Luke’s has helped patients successfully transition from hospital to home for generations. Our new St.

Luke’s Transitional Care Center is helping patients recover from illness or surgery and go home faster. Sometimes after an injury, surgery, or illness you no longer need to remain in the hospital but you are not confident or strong enough to return to your home. We are your bridge between a hospital stay and going home so that when you do go home you are successful and have a reduced chance of reh

ospitalization. Our caregivers and staff are committed to delivering the care you need when you need it. Our highly-skilled team of providers, nurses, social workers, and therapists design a personalized care plan for you with input and communication from the hospital doctors and your primary doctor. From daily visits by a nurse practitioner to a custom rehabilitation program geared at strengthening the body, all services are uniquely tailored to help you meet your individual health goals. Following a qualifying hospital stay, our services may be covered by Medicare. Private pay is also accepted for individuals wanting skilled nursing services for a short time prior to returning home.

🎉 Congratulations to our CNA students!

Today, our CNA students successfully completed their CNA class and clinicals! We are so proud of all the hard work, dedication, and compassion they have shown throughout their training.
We wish each of you the very best of luck as you take your CNA exam. Your future patients will be fortunate to have such caring individuals providing their care. Congratulations on this exciting milestone, and we can't wait to see all that you accomplish!
