Every year, our staff go above and beyond to create realism in patient simulation during our unhoused patient scenarios. Their commitment to accurate character development gives our students opportunities to hone their skills when it comes to understanding the special social, psychological, and medical situations that are unique to living unhoused. Compassion and understanding are introduced as key tools to pair with a thorough professional assessment and care plan.

Don’t lose yourself trying to fit into a profession that was never supposed to be your entire personality.

There’s something beautiful about fall. Every tree changes differently. Different colors, different timelines, different ways of letting go. And we don’t expect them all to look the same.

I wish we gave people that same freedom in EMS.

Somewhere along the way, we created this expectation that being a good paramedic means looking, acting, and thinking a certain way. That you have to fit the culture, share the same humor, and make this job your entire identity.

But EMS is what you do. It is not the entirety of who you are.

You’re allowed to be soft in a profession that sometimes celebrates being hardened. You’re allowed to be colorful in a culture that prefers everyone to blend in. You can be goofy, emotional, outspoken, quiet, or completely unconventional and still be a damn good paramedic.

Competence isn’t measured by how well you fit the mold. It’s measured by your knowledge, judgment, compassion, and willingness to grow.

And you’re allowed to have a life outside of this job. Different interests, different dreams, and an identity that extends far beyond the patch on your shoulder.

Because one day, the uniform comes off, the radio goes quiet, and the person underneath it all still deserves a life they love.

So be a good paramedic. But please, be a whole person, too.

A forest wouldn’t be nearly as beautiful if every tree looked the same. Neither would EMS.

I’m not throwing out the cervical collar. But I am using it a lot less.

Collars and SMR have been standard care for decades, yet no randomized controlled trial has ever studied them. Last year, NAEMSP’s Trauma Compendium (Millin et al.) found limited evidence supporting current practice.

This month, ACEP’s President and EMS Committee Chair responded in Prehospital Emergency Care. They agree the evidence is thin, and they raise fair cautions. Not finding evidence of harm doesn’t prove harm never happens. Rare, catastrophic injuries are hard to capture in a literature review. They’re calling for formally graded evidence and one joint guideline from NAEMSP, ACEP, ACS-COT, AAP, and the AANS/CNS Neurotrauma Section.

I think both sides have a point. Here’s where I’ve landed as a medical director.

We’re expanding field clearance. If a patient meets clearance criteria, they don’t need a collar. That spares a lot of people discomfort, unnecessary imaging, and time on a board for no benefit.

But I’m not ready to eliminate collars entirely. For the patients we can’t clear, I want that safety margin until the evidence is formally graded and our specialty partners are at the table.

That isn’t hedging. It’s following the evidence as far as it goes, and no further.

For those of you leading systems: are you clearing in the field, eliminating collars, or holding steady?
