Read that one again.

You can care that someone is disappointed without making their disappointment your responsibility.

You can care that someone is struggling without taking ownership of fixing every part of the struggle.

You can care that someone is upset with you without abandoning yourself to make their discomfort disappear.

For caring people, this distinction can feel enormous.

Because over-responsibility often looks compassionate from the outside.

Until you realize how much of everyone else's emotional world you've begun managing.

Caring deeply is not the same as carrying deeply.

Leadership capacity isn't only about skill.

It's also about how much internal bandwidth is available to access that skill.

You can still know exactly what to do and notice:

Less patience.
More reactivity.
Heavier decisions.
Reduced creativity.
Less room for connection.
More urgency.

Your competence may not have changed.

Your available capacity may have.

And when too much capacity is being consumed by accumulated emotional weight, it becomes much easier to lead from urgency rather than vision.

That is not a conversation about weakness.

It is a conversation about capacity.

Systemic problems do not stay “systemic.”

They land somewhere.

Staffing shortages land on the nurse trying to cover more patients.

Productivity demands land on the clinician deciding how much time they can give someone who needs more.

Bureaucracy lands on the physician still completing documentation after everyone else has gone home.

Organizational instability lands on the leader expected to keep the team steady.

These are some of the themes reflected in Suck It Up, Buttercup: Trust and Betrayal — Healthcare in America.

System change matters.

And so does what those systems leave behind inside the human being working within them.

But did work leave you?

Your body may be sitting at dinner, driving home or lying in bed while your mind is still:

Replaying the conversation.

Rechecking the decision.

Wondering whether you said too much.

Wondering whether you said enough.

Thinking about the person you couldn't help.

Preparing for tomorrow before today has ended.

That is a form of emotional and mental carrying.

And when it happens often enough, it begins to feel normal.

But normal doesn't necessarily mean sustainable.

For years, I believed strength meant continuing to carry it.

Push harder.

Stay composed.

Keep going.

Take care of everyone else.

I was very good at it.

Until I began to understand that strength and endless carrying are not the same thing.

That realization became deeply personal for me through nursing, funeral service, caregiving, grief, divorce and eventually the work I do today.

No one had taught me what to do with everything I absorbed while caring.

So I carried it.

That experience is part of why I now teach this work.

Not to make caring people care less.

To help them discover that there is another way to care.

One of the most dangerous places for a high performer to hide is inside their own competence.

You're still getting things done.

So you tell yourself you're fine.

Everyone around you may tell themselves the same thing.

But functioning tells me very little about what the functioning is costing you.

You can be successful and depleted.

Dependable and overwhelmed.

Compassionate and emotionally overloaded.

Strong and running out of capacity.

That is why I don't begin with, “Are you still managing?”

I want to know:

What are you carrying while you manage it?

Sometimes you still love the work.

That's what makes this conversation so important.

The problem isn't necessarily that you've stopped caring.

It may be that there has been so little space between the moments that require something from you.

No pause after the difficult conversation.

No recovery after the crisis.

No processing before the next person needs you.

No place for the grief, worry, responsibility or pressure to go.

And eventually, carrying becomes so normal that you may not even recognize it as carrying anymore.

Meeting to meeting.
Decision to decision.
Crisis to crisis.

And somewhere in between, clinicians are expected to simply keep moving.

This is one reason the conversation raised in Suck It Up, Buttercup: Trust and Betrayal — Healthcare in America matters.

Corporate pressure, bureaucracy, burnout, resource constraints and systemic dysfunction do not remain neatly contained inside “the system.”

People carry the impact of the system.

That doesn't mean the solution is to place responsibility back on the individual. Healthcare absolutely needs systemic change.

And while those changes are being fought for, we also cannot ignore the clinician whose nervous system has been absorbing the consequences day after day.
