May's DSC-Denver Senior Coalitions meeting at Encompass Health Littleton was another great example of what happens when professionals from across the continuum of care come together to learn and collaborate.
The morning featured an educational presentation on stroke rehabilitation and recovery, followed by an insightful discussion from Colorado HomeDoc on supporting stroke survivors after they return home. Together, the presentations highlighted the importance of coordinated care and the many professionals working behind the scenes to help individuals achieve the best possible outcomes.
