When life gets overwhelming, managing chronic conditions can easily fall to the back burner. That’s where community pharmacy steps in. 🩷
Through a Medicaid-funded asthma program, McCracken Pharmacy, a PPCN pharmacy, supported a 21-year-old pregnant patient who was struggling to manage her asthma while also navigating COVID. She hadn’t filled her inhalers in some time and didn’t have an active connection with her current P*P.
With a simple conversation, the pharmacy team uncovered gaps in care, reviewed proper inhaler technique, and coordinated directly with her provider to get her back on track. Within days, she had a follow-up appointment, updated medications, and a clearer plan.
At follow-up, she reported improved breathing and confidence in managing her symptoms, just from having the right support system in place.
This is the power of pharmacist-led care: meeting patients where they are, removing barriers, and helping them breathe a little easier—literally.
