🧬 EGPA and Hypereosinophilic Syndrome: Why IL-5 Matters

Eosinophilic granulomatosis with polyangiitis (EGPA) and hypereosinophilic syndrome (HES) are rare eosinophilic disorders associated with multi-organ damage.

At the center of both diseases: activated eosinophils.

Key mechanistic insights:

• Cytotoxic granule protein release → tissue injury
• IL-5 regulates eosinophil maturation, mobilization, and survival
• Tissue eosinophils do not recirculate and persist via local cytokine support
• Activated eosinophils contribute to endothelial dysfunction and thrombotic risk

EGPA phenotype distinction:

🔹 ANCA-positive → vasculitic features
🔹 ANCA-negative → eosinophil-driven tissue infiltration

Mechanism-based therapies targeting IL-5 or its receptor are approved or in development.
