Investing in leadership that strengthens our community.

Our CoHeWo managers joined professionals from across the Sacramento region at the Sacramento Metro Chamber EDGE EMERGE Summit focused on leadership, collaboration, and connection.

We value opportunities that support growth, learning, and leadership development as we continue building a strong and sustainable organization rooted in community impact.

Our offices will be closed on Monday, May 25 in observance of Memorial Day.

This Asian American and Pacific Islander Heritage Month, we're celebrating the members of our team who bring lived experience and cultural understanding to the communities we serve every day.

Research shows that health systems work better when the people navigating them reflect the communities they serve. Our staff of Community Health Workers offers services in 12 languages and meets people where they are, making care more accessible, culturally responsive, and effective.

That's the model Community HealthWorks was built on, and why we proudly advocate for community-based care.

Mental health is shaped by a range of factors.

For many of the people we serve, mental health might be connected to housing instability, other health issues, gaps in healthcare coverage, or difficulty navigating complexities in the system.

That's why our Community Health Workers show up every day, meeting people where they are and supporting them on a path that suits their unique needs.

During Mental Health Awareness Month, we celebrate our CHWs for their essential role in connecting people to care, housing, and services that support long-term stability and well-being.

Big Day of Giving 2026 showed us, once again, what’s possible when a community comes together.

To everyone who donated, shared our posts, cheered us on, and believes in this work — thank you. Your generosity helps ensure families have access to the resources they need to stay healthy, supported, and empowered.

Together, we’re creating lasting impact.

It keeps the lights on.
It puts a safe car seat in the back of a new parent’s car.
It gets someone to a critical doctor’s appointment.

Today is Big Day of Giving, and every gift counts.

Your donation right now directly supports families with the essentials they need to stay healthy, housed, and hopeful.

There’s still time to make an impact before midnight.
