We're launching something new...but it's a conversation long overdue.

In the world of AI, 'ground truth' represents verified, real-world data: the actual answer you check a model against.

In community health, ground truth is what's happening at the doorstep — the health worker, the patient, the conversation that happens in a village far from any boardroom or conference.

AI is being defined right now. Its purpose, its limits, and who gets to shape it. Most of that conversation is happening far from the people closest to care.

That's what Ground Truth is about: bringing it back.

Starting in June, we're sitting down with community health workers, implementers, technologists, funders, and the builders working at the last mile to ask what AI really needs to learn if it's going to unlock health for all.

We know the questions around AI don't have a single, one-size-fits-all answer. But we'll be sharing honest conversations that bring us one step closer. The kind you can only have when people with genuinely different vantage points are given space to think out loud.

This is Simon Pierre Gabo Gahou.

Since 2018, he has cycled through Gagnoa, south-central Côte d’Ivoire, caring for 60 households as a community health worker (CHW).

He kept track of every family, every pregnancy, and every child on paper.

That is until a Community Health Toolkit-powered app called SanteComCIV changed everything.

Now, Simon Pierre has a tool that guides disease diagnosis and referral, flags who needs urgent care, and gives recognition and visibility to those like him on the frontlines of care.

“Everything is quicker with the application,” he says.

The system is now deployed across nearly half the country. Over 8,200 CHWs like Simon Pierre use it to deliver quality care to 6.3 million people.

And the results tell an incredible story: 94% of supervisors say that the tool supports better clinical decision-making, and over 90% of households reported increased use of health services.

This is what transformative tech looks like when it’s developed with the communities it serves.

When communities shape the technology they use, health systems grow stronger.

That’s the core belief behind the CHT Community of Practice. And, in 2025, it was affirmed in amazing ways.

We welcomed 262 new members—a remarkable 344% increase from the previous year.

Contributions doubled, connections between members grew fivefold. A challenge in Kenya fuelled progress in Mali and a breakthrough in Nepal drove change in Jamaica.

This is the power of community: a global ecosystem driving transformation, together.
