Earlier this year, Monogram embarked on a clinical leadership series in to discuss what it truly means to provide in-home, evidence-based, multispecialty treatment for patients with multiple progressed chronic conditions.

Throughout the series, readers heard from our Chief Medical Officer, Dr. Shaminder Gupta, as well as Monogram’s Multispecialty Practice Chiefs: Dr. Susan Bell (Cardiology), Dr. Lakshman Swamy (Pulmonology), Dr. Zahily Fals (Endocrinology), Dr. Mindy Banks (Nephrology), and Dr. Alan Chiu (Palliative Care).

Each specialist wrote about diagnosing, treating, and prescribing from their vantage point, often integrating their treatment plans with those of their interdisciplinary colleagues. What has become abundantly clear throughout this clinical series is that true progress in chronic care starts when treatment accounts for a patient’s full medical history and the real-world factors that shape their health, not just their symptoms.

At the end of July, Monogram Health gathered 300 of its Advanced Practice Providers, CPLs, and Physicians at the Grand Hyatt in Downtown Nashville for Monogram’s Annual Clinical Summit. We compiled the recap video below, which captured the energy, clinical passion, and genuine connection surrounding the event.

The University of Pennsylvania’s Leonard Davis Institute of Health Economics and Monogram Health have launched a second research initiative to improve care for patients with multiple chronic conditions. The Penn LDI Research Program on Care for Medically Complex Patients will focus on generating new knowledge to inform improvements in the management of medically complex patients.

Monogram Health officially announces its partnership with Aetna, a CVS Health Company, has expanded to Washington State. Now, all eligible Aetna Medicare Advantage members across the state, who meet the program’s criteria, will benefit from Monogram’s value-based, in-home, multispecialty treatment and care for patients with multiple chronic conditions such as diabetes, hypertension, chronic kidney disease, heart failure, depression, COPD, and other metabolic disorders.

While Monogram Health does not replace the primary care relationships for patients, we can serve as an additional resource and extension of the patient’s current care team that can treat and diagnose health issues; review and prescribe medication; provides guidance, education, and counseling on patients’ healthcare options; as well as assists with daily needs such as access to food, eating healthy, transportation, financial assistance, and more. Monogram Health has a proven track record for driving positive clinical outcomes, resulting in reduced medical costs and a high quality of living for all engaged patients.

To date, all eligible Washington-based Aetna patients will have 24/7 access to in-home, personalized treatment and care from doctors, nurse practitioners, specialists, pharmacists, registered nurses, and social workers. Since 2023, Monogram Health has successfully collaborated with Aetna, a CVS Health Company, to care for more than 75,000 attributed patients across 9 states - California, Florida, Iowa, New Jersey, Pennsylvania, South Carolina, Texas, Washington State, and West Virginia.

NCQA - National Committee for Quality Assurance has renewed Monogram’s Case Management Accreditation for three years.

Since 2023, Monogram has continued to demonstrate strong performance in areas such as comprehensive needs assessment, individualized care planning, interdisciplinary collaboration, timely follow-up, and measurement of outcomes which has met and/or exceeded NCQA’s criteria for managing care across diverse populations and complex conditions.

Today, Monogram Health announced it has renewed its partnership with the Leonard Davis Institute of Health Economics at the University of Pennsylvania to support independent, academically rigorous evaluation of care models for individuals with complex, progressed chronic disease and test how multidisciplinary, continuous, and home-based approaches can impact patient outcomes, patient experience, costs, and overall system burden.

Today, Monogram’s Chief of Palliative Care, Alan Chiu, published an article in Becker's Healthcare, entitled “Redefining Upstream Palliative Care for the Patient and the Industry.”

In his article, Dr. Chiu breaks down misconceptions about palliative care, compared with hospice care and end-of-life care, and connects Monogram’s palliative practices to geriatric medicine principles. Dr. Chiu outlines the industry's challenges and highlights best practices/solutions using Monogram’s model, which includes foundational palliative care practices/concepts.
