Monogram Health is thrilled to announce our partnership with Aetna, a CVS Health Company, has expanded to West Virginia. Now, all eligible Aetna Medicare Advantage members across the state, who meet the program’s criteria, will benefit from Monogram’s value-based, in-home, multispecialty treatment and care for patients with multiple chronic conditions, such as diabetes, hypertension, chronic kidney disease, heart failure, depression, COPD, and other metabolic disorders. While Monogram Health isn't a substitute for your primary care, we can serve as an additional resource and extension of your current care team.

Like many states, West Virginia faces a shortage of healthcare professionals, rising healthcare costs, and the need to expand interventional services. Monogram Health has a proven track record of driving positive clinical outcomes, reducing hospital and ER admission rates, and increasing patient ratings and reviews, resulting in reduced medical costs and a high quality of life for all engaged patients.

Since 2023, Monogram Health has successfully collaborated with Aetna, a CVS Health Company, to care for more than 53,000 attributed patients across Florida, Iowa, New Jersey, Pennsylvania, Texas - and now West Virginia - with a goal of expanding to more states shortly.

Mr. Thomas Hutto lives on his own in Garrison, Texas (population 846). Mr. Hutto has advanced COPD, congestive heart failure, and CKD. Before Monogram, Mr. Hutto had to travel 2+ hours to see a specialist or visit the nearest “walk-in only” Urgent Care for any exacerbations he was experiencing or to get his medications refilled/reviewed. Since Monogram, Mr. Hutto has been able to stay in his home and receive the multispecialty treatment and care he needs on his time. He now has direct access to clinical tests, pharmacists, nurses, doctors, and specialists 24/7 at his kitchen table.

May is Mental Health Awareness Month. Mental health conditions are highly prevalent across the populations we serve, particularly among patients with multiple chronic conditions. More than 1 in 5 U.S. adults experience a mental illness each year, and nearly 1 in 3 adults with a chronic medical condition also have a co-occurring behavioral health condition such as depression or anxiety. Individuals with polychronic disease carry a disproportionate burden of these challenges, with depression rates 2-3 times higher than in the general population.

This May, we acknowledge this reality and continue to treat and care for the whole patient 24/7.
