The current state of rheumatology is a “lose-lose.” Patients wait months for minutes of care, and doctors are burnt out by a system that values codes over people.

We decided to stop playing that game.

Does diabetes increase your risk of gout?

Not directly.

Diabetes itself isn’t technically a cause of gout but the two often show up together.

Why?

Because they share common risk factors:

• Insulin resistance
• Overweight or metabolic syndrome
• Diets high in purines (processed meats, red meat, beer)
• Reduced kidney function

When kidney function declines, uric acid doesn’t clear as efficiently and that’s what can trigger gout flares.

So it’s less about diabetes “causing” gout…
and more about overlapping metabolic pathways.

This is why treating the whole metabolic picture matters, not just one diagnosis at a time.

Here’s the reality:
Patients are waiting 3–6 months to be seen. Visits are rushed. Doctors are overwhelmed.

And then institutions lower compensation… and we’re surprised fewer trainees choose rheumatology?

We’re asking physicians to train 2–3 extra years to earn less than they would without subspecializing and wondering why the pipeline is shrinking.

If we care about access to care, we have to care about the system that supports it.

In the meantime, I have space. And I will make space for you if you need thoughtful, unhurried rheumatology care.
