September is National Ovarian Cancer Awareness Month. The signs can be subtle—bloating, pelvic pain, feeling full too quickly, urinary changes, or fatigue. Because they seem “normal,” many women dismiss them as aging, diet, or stress. But these vague symptoms can be early warning signals. Help us give a warm welcome to the newest member of the Community Care Internal Medicine – Delmar team! 👋💙

We’re excited to welcome Creig Hayes, FNP-C, to our team as a Family Nurse Practitioner providing adult primary care!

Creig is passionate about prevention, nutrition, exercise, and overall wellness. With a clinical background in cardiac care, he takes a whole-person approach to health and enjoys helping patients make sustainable lifestyle changes that support their long-term well-being.

His path to healthcare started in an unexpected place. While initially pursuing a degree in physical education, Creig discovered that his true passion was helping others. That realization led him to nursing and eventually to becoming a Family Nurse Practitioner. He earned his undergraduate nursing degree from Excelsior College and completed his graduate education at SUNY Polytechnic Institute.

Originally from Berlin, NY, Creig has made the Delmar area his home, where he is building a life and raising his family. He values caring for community members and building meaningful, long-term relationships with his patients.

Outside of the office, Creig enjoys staying active and maintaining a healthy lifestyle. His experience with a variety of fitness activities has helped shape his balanced approach to health and wellness, both personally and professionally. He enjoys bringing that perspective into conversations with patients about building healthy habits into their everyday lives. 💪🥗

When it comes to patient care, Creig believes in taking the time to listen, educate, and work together. His goal is to make sure every patient feels heard, respected, and involved in their healthcare decisions.

🎒 Back-to-school nerves are normal, but when should parents be concerned?

A new school year can bring plenty of emotions for kids. Meeting new teachers, adjusting to new routines, homework, friendships, and academic expectations can all cause some anxiety.

But if worry starts to interfere with your child’s everyday life, it may be time to take a closer look. 🚩

Keep an eye out for signs such as:
• Frequent stomachaches or headaches with no clear medical cause
• Trouble sleeping or changes in appetite
• Avoiding school or activities they normally enjoy
• Excessive worry about school, grades, friends, or making mistakes
• Increased irritability, crying, or difficulty concentrating
• Physical symptoms that appear before school or school-related activities

💬 The most important thing? Listen. Let your child know their feelings are valid and that they don’t have to navigate them alone. Talking with your child’s pediatrician can also help you understand what they’re experiencing and determine what support may be appropriate.

Every child handles transitions differently. A little extra reassurance and an open conversation can go a long way. 💙

Please join us in welcoming Madison Blood, PA-C, to Community Care Family Medicine!

Madison is a Physician Assistant who is passionate about family medicine and providing comprehensive, patient-centered care. She believes that great healthcare starts with understanding the whole person and building strong, lasting relationships with patients.

Her approach focuses on preventive care, education, wellness, and shared decision-making, helping patients feel informed and empowered about their health. She has a particular interest in family medicine and promoting overall wellness, while taking each patient’s individual preferences, lifestyle, and medical history into account.

Madison earned her Bachelor of Science in Biotechnology from SUNY Cobleskill and her Master of Science in Physician Assistant Studies from St. Bonaventure University. During her clinical training, she completed elective rotations in hematology-oncology and outpatient neurology.

Before becoming a PA, Madison worked as an EMT, where she discovered her passion for helping others and providing compassionate care. That experience helped inspire her path to family medicine, a specialty that allows her to care for patients across every stage of life.

Outside of work, Madison enjoys hiking, kayaking, and camping throughout the Capital Region. She also supports the National Multiple Sclerosis Society and hopes to visit all of America’s national parks one day!

☀️ It's still warm outside, but believe it or not, flu season is right around the corner! And we're already hearing from patients asking when flu vaccines will be available at their CCP practice. You're smart to start thinking ahead!

Good news: our primary care practices have started to receive their flu vaccine supplies.

Keep an eye on your email! Your CCP practice will share information about when you can get your flu vaccine, upcoming flu clinic dates (if offered), and how to schedule. Many offices offer online scheduling for your convenience, so take advantage of it when available. If your office offers online scheduling, you'll receive the link in your email. You can also get your flu shot during a regularly scheduled appointment.

Your vaccination is immediately documented in your medical record, and we offer age-appropriate flu vaccines based on the latest clinical guidance.

Why get vaccinated? 💉
The CDC recommends an annual flu vaccine for everyone 6 months and older. It's one of the best ways to reduce your risk of getting the flu and, importantly, reduce your risk of becoming seriously ill if you do get sick.

Vaccination is especially important for adults 65 and older, pregnant women, people with chronic health conditions such as asthma, diabetes, or heart disease, and those who live with or care for people at higher risk for serious illness.

And remember, if you have questions about the flu vaccine, you can always talk with your CCP provider.

We’re thrilled to welcome Dr. Forti, a board-certified family medicine physician who is passionate about providing compassionate, comprehensive care for patients at every stage of life.

Dr. Forti believes that building lasting relationships with her patients, listening carefully, and maintaining open communication are key to providing exceptional care. She especially enjoys caring for older adults, managing complex medical conditions, and partnering with patients to help them achieve their personal health goals.

A Capital Region native from Knox, NY, Dr. Forti is excited to return to the area after completing her medical training. She earned her medical degree from SUNY Upstate Medical University and completed her Family Medicine residency at Northwell Health Plainview Family Medicine Residency.

🚲 E-bikes for kids: yay or nay?

E-bikes can be a fun way for kids and teens to get around, but they also bring added safety concerns because of their speed and weight. The American Academy of Pediatrics recommends that parents consider their child’s age, maturity, riding experience, and local laws before deciding if an e-bike is appropriate.

Our CCP pediatricians agree that this is a decision parents should take seriously.

Dr. Deimling from Community Care Pediatrics - Latham shares a simple rule of thumb:

“Our general rule - avoid! These go VERY fast, and may be in the hands of too young, unrestrained, and sometimes non-helmeted children.”

Dr. Cirenza from Community Care Pediatrics - Saratoga recommends thinking about an e-bike much like a motorcycle:

“Parents should treat an e-bike as they would a motorcycle, and so the risks are very similar. If you don’t think your child is ready for a motorcycle, I would seriously consider their readiness for an e-bike as not quite ready.”

He also points out that the potential for serious injury goes beyond how safely a child rides. E-bikes are often used on streets with cars and can reach speeds that may result in significant injuries. Even when a child follows the rules, another driver may not.

A good rule of thumb? Dr. Cirenza recommends that a child should first prove their readiness on a traditional bike, without a motor, before an e-bike is even considered.

Please join us in welcoming Elisabeth “Lisa” LaBrie, MSN, FNP-BC, to CapitalCare Family Practice River Road! 💜

Lisa is a board-certified Family Nurse Practitioner dedicated to providing comprehensive primary care for patients of all ages. She is passionate about building lasting relationships with her patients and providing personalized, preventive care that supports lifelong health and wellness.

Lisa began her nursing career as an ICU registered nurse, where she saw firsthand how gaps in preventive and primary care could contribute to serious health conditions. That experience inspired her to continue her education and pursue a career focused on helping patients stay healthy, manage chronic conditions, and take a proactive approach to their care.

Her educational journey began at Samaritan Hospital School of Nursing, where she earned her Associate of Science in Nursing. She went on to earn her Bachelor of Science in Nursing through SUNY Delhi’s RN-to-BSN program before completing her Master of Science in Nursing as a Family Nurse Practitioner at Binghamton University, State University of New York. Her clinical training included experience in women’s health, pediatrics, and internal medicine.

Lisa has a particular interest in chronic disease management and women’s health. She believes that great healthcare is a partnership and enjoys helping patients better understand their health, make informed decisions, and work toward their individual wellness goals. Her favorite part of practicing medicine is building trusting relationships with patients and advocating for them throughout their healthcare journey.

A lifelong resident of New York’s Capital Region, Lisa is especially grateful for the opportunity to care for the community she calls home. She believes one of the most important parts of healthcare is empowering patients to advocate for themselves and take an active role in their care.

Lisa is a member of the American Nurses Association and the American Nurses Credentialing Center (ANA-ANCC).

We’re excited to have Lisa join the CapitalCare Family Practice River Road team and look forward to the compassionate, patient-centered care she will provide to our community.
