🎉 A Transformational Milestone for Our Community 🎉

Community Access Network has been selected to receive federal Health Center funding through the Health Resources and Services Administration's (HRSA) New Access Point program — officially transitioning CAN to a federally funded Federally Qualified Health Center!

This award is more than funding. It's federal recognition of the care our staff provides every day, the strength of our patient-governed model, and more than a decade of work removing barriers so that everyone in our community can access compassionate, high-quality healthcare.

💬 "This is a transformational moment for Community Access Network and for the communities we serve," said Christina Delzingaro, Chief Executive Officer of Community Access Network. "This award affirms the work our staff, board members, patients, and community partners have done together to create a model of care centered on dignity, access, and health equity."

🤝 A New Location at Jubilee Family Development Center
As part of this award, CAN plans to open a satellite site at Jubilee Family Development Center on Florida Avenue, anticipated to open in late November 2026. The site will offer:

🩺 Primary care — by appointment and walk-in
🤗 Support from a community health worker
🧠 Access to behavioral health services through telehealth
🔗 Connection to CAN's full network of services

Jubilee is a trusted partner in a historic neighborhood, and this partnership allows us to bring care into a setting our neighbors already know and trust.
