When families choose Commonheart Hospice, they’re placing their trust in our team to provide comfort, compassion, and personalized support.

We’re honored to walk alongside families and assist each person’s unique journey with thoughtful, individualized care. Thank you to Kristine L. for sharing this meaningful reflection on her mother's experience with Commonheart Hospice.

We are grateful for the opportunity to give back to those who have given so much for all of us.

Commonheart was honored to sponsor Capri Communities annual Dream Flights experience at Waukesha County Airport/Crites Field, giving 15 local veterans the opportunity to take to the skies aboard a historic biplane for a ride of a lifetime.

Thanks to John Wick for representing Commonheart at an event filled with joy and meaningful moments for these incredible heroes.

We are deeply thankful to each and every veteran for their courage, service, and sacrifice.

As the 2026 Commonheart Sporting Clay Shootout draws closer, we’re reminded of the people and stories at the heart of the Commonheart mission.

Michael was a U.S. Navy Veteran who received Hospice care from Commonheart. He spent his life serving others, including helping bring The Moving Wall Vietnam Memorial to Watertown. His life story is a beautiful reminder of the impact one person can have on a community—and of the importance of being there for people when they need care and support most.

When you donate to Commonheart, you help make compassionate, person-centered care possible for patients and families during some of life’s most difficult moments. Your support helps Commonheart continue to care for the whole person, honoring their stories, their lives, and the people who matter most to them.

You can support the Commonheart mission by making a donation directly to the Illuminus Foundation: Commonheart Fund. Learn more about the Foundation and how your gift can make a difference: https://www.illuminus.us/donating

September often symbolizes change, and it's a crucial time in your grief journey. Chaplain Thomas Moe encourages reflection on how you've grown and adapted through this challenging process. We're reminded not to hastily move on from the past, but to cherish it while embracing new changes. He also highlights the importance of not overscheduling, adapting to seasonal shifts, and finding supportive communities. Watch his commentary through the link below.

Today is National Grief Awareness Day.

Grief looks different for everyone, and wherever you are on your journey, we want to meet you where you’re at with the support and resources you need.

That’s why we created the Commonheart Grief Series—a collection of videos featuring our Chaplains covering a variety of grief-related topics to help provide guidance, encouragement, and support along the way.

If you or someone you know is navigating grief, we invite you to explore our series and find resources that meet you where you are. You don’t have to walk this journey alone.

A heartwarming shared moment: During a hospice visit with social worker Julie Pryce, a family shared what they hoped to experience together before their beloved dad, Rodrigo, passes. One of those wishes came from his daughter, Sam. She wanted to go to a bridal shop and try on wedding dresses - not because she is engaged, but simply because she didn’t want to miss the opportunity to share that special experience with her dad and mom.

So, we helped make that dream a reality. With the help of Sandra D's Bridal Boutique in Watertown, Julie arranged for Sam to try on dresses and share the moment with Rodrigo and Sam’s mom, Jodi. Julie attended and they were pleasantly surprised and very grateful for the gesture. We also provided $300 in gift cards from the Because We Care program, to help them enjoy additional time together.

A heartfelt thanks to Emily at Sandra D’s and to Julie, for seeing this family’s wish, helping coordinate the details, and going above and beyond to make this special day happen. Your compassion and dedication make a lasting difference.

When someone receives home health care, the support extends beyond the patient. Home health teams provide education, training, and guidance for family caregivers, helping them feel more confident and supported every step of the way.

Caring for a loved one is easier when no one has to do it alone. Allow Commonheart to support you and your loved one on their health care journey.
