Filler is one of the most misunderstood treatments in aesthetics, and the concerns people have about it are almost always based on what they’ve seen go wrong elsewhere.

Fear of looking fake, the assumption it’s about making features bigger, the belief that it’s permanent. These are the three I hear almost every day in clinic, and none of them reflects what filler actually is when it’s used properly.

When placed thoughtfully and strategically, filler is about restoring what time has gradually taken, in amounts that are almost always smaller than people expect, with results that are virtually undetectable. And for most hyaluronic acid fillers, the commitment isn’t permanent either.

A good practitioner will always be honest when filler isn’t the answer, and help you understand the best approach for your skin and overall treatment goals.

The single biggest mistake I see when it comes to jowls, and it happens more than it should, is people being treated at the jawline for a problem that did not start there.

Filler placed directly into a jowl does not lift the face. It adds volume to an area that already has too much tissue sitting in the wrong place. The result is a heavier lower face, not a lifted one.

Jowling is a whole-face problem. The descent you see at the jaw is the end point of a process that started much higher up, with bone loss, fat pad descent, weakening ligaments, and reduced collagen in the mid-face and temples. Treat the cause, not just what you can see in the mirror, and the result is something that actually makes sense anatomically.

This is why when a patient points at their jaw in my consultation room, I usually start by looking at their midface.

Sun damaged skin is one of the most common concerns I see in clinic, and most people are surprised when I tell them where I’d actually start.

The instinct is usually to jump straight to lasers, peels, or injectables. And while those treatments absolutely have their place, they work significantly better on skin that’s already in good health. Skipping the foundation and going straight to the bigger guns is one of the most common reasons people don’t get the results they were hoping for.

The starting point is always stopping ongoing damage and getting the skin into the best possible condition first. That means consistent SPF, the right active ingredients introduced properly and at the right pace, and giving the skin barrier the support it needs to function well. Healthy skin responds better to everything that comes after it, and building a treatment plan in layers rather than all at once is almost always what delivers the most lasting result.

The aesthetics industry moves fast - new treatments, new trends, new products appearing constantly - and somewhere in all of that, it's easy to lose sight of what actually matters: a result that looks natural, skin that is properly cared for, and a patient who genuinely understands what they are doing and why.

I will not treat a face the way someone points at it in the mirror. I will look at the whole picture, the structure, the skin, the long-term plan, and make a recommendation based on what will actually serve that person best.

Sometimes that means starting with skincare before we touch anything else. Sometimes it means having an honest conversation about what is and is not going to make a meaningful difference.

That is the standard I hold every consultation to. And it is the reason patients come back not because they feel they have to, but because they trust the process and they can see it working.

The mouth area often ages faster because it’s doing more work than almost anywhere else on the face.

We talk, smile, eat, laugh, drink through straws, purse our lips, express emotion, all day, every day. Over time, that repeated movement starts to show in the skin and the supporting structures around it.

But movement is only one part of the picture. As we age, we also lose collagen, fat, hydration and support around the lips, chin and lower face. The lips can become flatter or less defined, the skin around the mouth can become more delicate, and the lines from the nose to the mouth or mouth to chin can start to look deeper.

This is why treating the mouth area well is rarely about just “adding filler”.

✨️Sometimes we need to restore subtle support to the lips or surrounding tissue.

If you’re serious about ageing well, vitamin A and SPF are the two products I’d start with - and they’re the most useful, well-supported ingredients we have for long-term skin health ✨️

Vitamin A is where a lot of people get confused. There isn’t just one “retinol”. There are different forms, from gentler options like retinyl palmitate, through to retinol, retinaldehyde and prescription-strength tretinoin. They all sit on the same pathway, but they’re not equally strong, and they’re definitely not equally tolerated.

That’s why choosing the right one matters.

Vitamin A can help with fine lines, pigmentation, texture, breakouts, collagen support and overall skin quality, but only if your skin can actually tolerate it. Starting too strong or using it too often is usually where irritation, peeling and barrier damage happen.

The goal isn’t to get to the strongest product as quickly as possible.

It’s to find the right strength for your skin, introduce it slowly, and use it consistently.

And then comes the non-negotiable: SPF. There’s very little point investing in active skincare if you’re not protecting your skin from UV damage every day. SPF is what helps protect collagen, reduce pigmentation risk, prevent premature ageing and support every other part of your routine.
