For those who have been contacted and/or have a new patient appointment scheduled at Perryman Rheumatology, we have a new process that must be completed and returned PRIOR to your new patient appointment date and/or time. You will have the option to have fillable pdf forms that you can do online and return via email OR we can snail mail the forms to you with a return envelope. Every new patient who is contacted for an appointment must choose what option works best for them and will receive a new patient info letter that states the form MUST be completed and returned at least 5 days prior to the appointment date/time AND the appointment MUST be confirmed or we will reschedule the appointment to another date/time. This is so important with our office, we have only 2 physicians...our new patient appointments are a wait..right now it is 6 to 7 months! Please follow our new expectations as we work so hard to make sure that we get you in the best and fastest time we can and our providers truly do give each patient the best listening, the best heart and the very best care!
