A Year of Partnership, Resilience, and Action

We’re proud to share our 2025 Annual Highlights report — offering a look at what we have learned and accomplished alongside the organizations, residents, and community leaders who are working every day to build healthier communities across our region.

In a year marked by uncertainty and changing funding environments, partners continued to adapt, collaborate, and respond to community needs. We were grateful for the opportunity to support that work through increased giving, the launch of our Rapid Response Fund, expanded co-funding partnerships, and resources that extend beyond grantmaking.

The stories in this report reflect the many ways communities are creating change; from expanding access to food, housing, health care, and maternal health resources to advancing racial health equity, supporting frontline workers, and strengthening rural communities.

They also include a mini-documentary celebrating the residents and organizations who have shaped healthier communities through the Neighborhood Health Status Improvement initiative for more than a decade.
